Patient care programs

There are five specialty care program areas offered at The Grace.

Critical Care Program

The Intensive Care Unit provides care to patients with unstable, critical conditions who require close observation and complex treatment. Critical Care areas are therefore staffed differently – usually one nurse caring for only one or two patients. Physicians, nurses and other professionals with special training are available 24 hours.  The unit has monitors and equipment for emergency and trauma care. Patient progress is discussed daily to decide when it is safe to transfer patients to other nursing units.

Emergency Program

Each year about 25,000 people come to our Emergency Department. Patients are assessed by a triage nurse and treated as quickly as possible. Patients who require close monitoring for a short while may be kept in the Observation Unit.

Medicine Program

The Medicine Program is responsible for providing a comprehensive range of acute inpatient and ambulatory care services to patients. Each year, more than 2500 individuals are admitted to one of the 3-inpatient units generating 35,000 patient days. In addition, 3500 individuals visit the medicine program’s Adult Medical Unit an outpatient clinic serving the community and Grace Hospital.

 

The Medicine Program provides patient care services in several settings:

Inpatient Units:

There are 3 inpatient units at the Grace Hospital – 3North, 5North, and 3South with a total of 101 inpatient beds. Cardiology patients, requiring telemetry monitoring, are admitted to either 3North or 3South.

3North - is a 30-bed Clinical Teaching Unit (CTU) that provides care for patients with unstable, acute medical conditions who require complex medical assessment, diagnosis, and therapeutic interventions. This CTU provides a clinical practice setting for University of Manitoba, Faculty of Medicine medical students and post-graduate medical physicians.

5North – is a 35-bed unit that provides care for patients with acute medical conditions and for those who are medically stable, in a sub-acute phase and continue to require hospitalization.

3South – is a 36-bed unit that provides care for patients with acute medical conditions and for those who are medically stable, in a sub-acute phase and continue to require hospitalization. This unit offers a 16-bed geriatric rehabilitation program for those individuals requiring extended convalescent hospitalization.

Adult Medical Clinic:

The Adult Medical Clinic opened in 2004 to provide general internal and subspecialty medicine resources for individuals who are able to safely return to their homes after an event requiring an Emergency department visit or an inpatient stay. The clinic also serves consultations from community family physicians. This outpatient clinic has linkages with community resources for patient access as required.

General Internal Medicine Clinics:

This clinic provides consultative services and follow up, if required, for those individuals with cardiovascular disease, chest pain, diabetes, and hypertension.

Neurology Clinics:

The neurology clinic provides consult services for assessment and follow-up, if required,  of individuals with neurological conditions i.e. strokes, seizures, and TIA’s.

Early Arthritis Clinics:

This clinic has been established to facilitate assessment of individuals with early inflammatory arthritis. Individuals with symptoms (joint pain, swelling, reduced joint mobility, and morning stiffness for greater than 30 minutes) can be referred for assessment and follow-up.

Allergy/Asthma Clinics:

Adults with allergies or asthma may be referred to this clinic for assessment, treatment recommendations and follow-up, if required.

Vascular Clinics and Vascular Lab:

This clinic was established to investigate the benefit of intensive cholesterol management coupled with concentrated walking clinics and education for individuals with identified vascular disease and uncontrolled hypertension.

CancerCare:

The clinic team responds to the varied care needs of close to 1,200 adults and their families living with cancer or blood-related disorders, Care focuses on support at various stages of illness including initial assessment and treatment decision-making, ongoing treatment provision, monitoring after the completion of active treatment, and support for individuals who are living with progressing and serious illness. All patients are encouraged to participate actively in care planning with various members of the team, and self-care that encompasses physical, social, emotional, and spiritual dimensions of their lives. Linkages with other community resources are also encouraged.

Hospice:

Opened in 2004, the Grace Hospice provides an environment of peace, compassion, and comfort. Specially trained staff and volunteers offer respectful, responsive care that reduces pain and suffering, while facilitating personal growth and healing in the last days of life.

Mental Health Program

A team of professionals with expertise in mental health help adults and their families deal with bio-psycho-social illness on an in-patient and out-patient basis. Psychiatric nurses provide care in the Emergency Department when patients and/or their significant others are in psychiatric distress.

Emotional support, counselling, psychotherapy and medication are used to help patients and their families within a safe environment. They learn crisis prevention and management, symptom control and problem solving of events in daily life.

Surgery Program

The Surgery Program has several departments:

Pre-Admission Clinic (PAC)

Nurses in this unit assess patients and identify health problems that may affect the surgery or recovery periods. They teach patients and families how to prepare for surgery, what to expect during their surgical experience and what they can do to help their recovery. Pre-operative tests may be done, and some patients will be seen by an anaesthetist, respiratory therapist, physiotherapist or occupational therapist during their visit. If required, specialists are consulted and will see the patient before surgery.

Surgical Day Care (SDC)

This unit prepares all patients for surgery and certain diagnostic procedures. Patients already in hospital are prepared on their unit. Patients who require less than 24 hours of care after surgery are cared for in SDC. They may be discharged the same day or early the next morning.  Twenty-four hour care (TFC) patients are SDC patients who need overnight care.  Patients needing longer recovery time are admitted to a surgical unit.

Operating Room

Approximately 10,000 surgical procedures are done each year at The Grace. Different types of surgeries are done at different hospitals in the city. At The Grace, surgeries most often performed are: orthopedics, urology, general surgery, neurosurgery, endoscopy, gynecology, plastic surgery, thoracic surgery, and oral, ear, nose and throat surgery.  Special tests that need to be done in the operating room are also performed.

Post Anaesthetic Care Unit (PACU) 

Patients who receive an anaesthetic are cared for in PACU by specially trained nurses. Any change in condition is quickly treated.

In-Patient Units

There are three in-patient units in the Surgery Program at the Grace Hospital – 2 South, 4 South and 4 East with a total of 96 beds.  A multidisciplinary team of health care professionals plans and provides care for those patients who require hospitalization for their surgical recovery.   

2 South is a 40 bed acute orthopedic unit that provides care for patients following orthopedic surgery such as total joint replacements, back surgery or repair of fractures.  Recovery from surgery and regaining strength and mobility is a priority on this unit.    

4 South is a 36 bed acute surgery unit that provides care for patients who have undergone major general surgery, urology, thoracic and gynecology procedures. 

4 East is a 20 bed orthopedic rehabilitation unit that provides care for patients who have had orthopedic surgery or have orthopedic injuries and require extended hospitalization for recovery.

Orthopedic Clinic

The Orthopedic Clinic is available for out-patient care of patients requiring special follow-up after receiving orthopedic services at the Grace Hospital.  This care is provided by orthopedic technologists under the direction of the orthopedic surgeon.   

 

Bereavement Care Services

Our Bereavement Care Service includes staff and volunteers who offer drop-in sessions, support groups, regular telephone calls and memorial services.  They also help individuals get in touch with other community resources that can help them through the grief experience. DSM Medical Laboratory Services

Laboratory services at Grace Hospital (GH) are operated by Diagnostic Services of Manitoba (DSM).

As part of DSM's provincial diagnostic network, the Medical Laboratory at Grace Hospital is a point of access for facility in-patients to a comprehensive range of laboratory services, including Biochemistry, Hematopathology, Immunology, Microbiology, Transfusion Medicine, Surgical Pathology, Autopsy and Cytology, for the purpose of diagnosis, pre and post operative testing and treatment monitoring. Emergency and in-patient services are offered 24 hours/day.

The lab also provides testing to several Personal Care Homes located within the area.

DSM's specimen collection centre is open to out-patients with requisitions from physicians at that facility. Please call the facility switchboard for hours: Tel 837-8311.

DSM is the not-for-profit corporation responsible for all of Manitoba's public laboratory and rural diagnostic imaging services. As an important part of DSM's provincial network the lab at Grace Hospital is one of 77 facilities working collaboratively to provide high-quality, state-of-the-art services to Manitoba's diagnostic customers.
