  • Ask for information and clarification that will help you and/or the person of your choice make decisions, accepting that these choices will enhance or limit the outcome of care
  • Follow the agreed upon treatment plan to the best of your ability
  • Tell us your expectations of care and of the health care team, recognizing the limitations of available health care resources
